Brazil business visa Type II Chicago Consulate

 

  1. Original passport (valid 6 months beyond stay with at least two clear unused visa pages)

  3. 1 passport photo full face white background (2 inches by 2 inches)

  4. Business visa applicants will need to submit a letter from their U.S. company explaining the purpose of their trip and providing a financial guarantee. If requesting a long term multiple entry visa (TYPE II) you must provide a letter stating that the purpose of the trip is for business meetings and is of a non-technical nature. Included in this letter must be the phrase "No technical assistance will be provided”

  5. Business visa applicants will also need to submit a letter of invitation from the company in Brazil that they are going to visit. A fax copy is acceptable. This letter also should state the purpose of the trip

  6. 1 Copy of flight itinerary (complete round trip)
  7. 1 Copy of Drivers License.
  8. Foreign passport holder: Copy of USA immigration status (green card H1 etc.)
  9. USA passport consulate fee for business Type II visa is $210 payable only with a “US Postal Svc. Money order”. If you do not provide the money order Peninsula Visa will charge $25 to obtain one for you.

  6. Foreign passport holder: Copy of USA immigration status (green card H1 etc.)
  7. USA passport consulate fee for Tourist visa is $150 payable only with a “US Postal Svc. Money order”. If you do not provide the money order Peninsula Visa will charge $25 to obtain one for you.

  8. Minors under 18 years of age:  
    . A copy of the minor’s birth certificate is required in all cases;
    . Minors not traveling with both parents/guardians must provide a notarized letter of consent signed by the non-accompanying parent(s)/guardian(s) authorizing the Consulate to issue a visa; 
    . A certificate of vaccination against polio is required for children between ages of three months and six years. A notarized letter from the child's physician is required if the child cannot be inoculated.
