Brazil business visa Type II Miami Consulate

1. Original passport (valid 6 months beyond stay with at least two clear unused visa pages)
2. 1 completed visa application form http://www.brazilmiami.org/eng/pdf/visa_form.pdf
3. 1 passport photo full face white background (2 inches by 2 inches)
4. Notarized letter from the employer of sponsoring US company, clearly stating the precise nature of business, dates of arrival and departure, and financial responsibility for the costs of the trip (the letter must be on an official company letterhead or stationary, signed by a senior manager, introducing the applicant and specifying his/her job/position in the company).
5. 1 Copy of flight itinerary (complete round trip)
6. 1 Copy of Drivers License
7. Foreign passport holder: Copy of USA immigration status (green card H1 etc.)
8. USA passport consulate fee for business Type II visa is $200 payable only with a “US Postal Svc. Money order”. If you do not provide the money order Peninsula Visa will charge $25 to obtain one for you.
9. Foreign passport holder fee (call Peninsula Visa)
10. Please enclose prepaid return air bill.
11. Processing time 10 business days in the Consulate.
12. Rush Processing 4 business days in the Consulate.

Brazil Tourist visa Miami Consulate

6. Foreign passport holder: Copy of USA immigration status (green card H1 etc.)
7. USA passport consulate fee for Tourist visa is $140 payable only with a “US Postal Svc. Money order”. If you do not provide the money order Peninsula Visa will charge $25 to obtain one for you.

8. Minors under 18 years of age:  
   . A copy of the minor’s birth certificate is required in all cases;
   . Minors not traveling with both parents/guardians must provide a notarized letter of consent signed by the non-accompanying parent(s)/guardian(s) authorizing the Consulate to issue a visa; 
   . A certificate of vaccination against polio is required for children between ages of three months and six years. A notarized letter from the child's physician is required if the child cannot be inoculated.
