Add Additional Pages to U. S. Passport
Requirements to Expedite
- Completed passport pages application form,(create your DS-4085 here .)
The form must have a bar code on the top left hand side of the form.
- Your valid U.S. passport.
- Authorization letter addressed: "Attn: U.S. Passport
Agency" and stating "I authorize Peninsula Visa Service to pick
up my passport and to discuss the status of my application with the
Passport Services staff." This letter must be signed by the
applicant. Print letter.
- Copy of itinerary of air tickets showing departure date from the
U.S.
- US State Department fee is $60 for Expedited Service.

Processing Instructions
Please deliver to our office or call for pickup of completed requirements. If
out of the area please call 1-800-665-8472 with departure and Fed-Ex air bill
number. This location accepts only Fed-Ex packages. You must mark
the Hold for Pickup box at the bottom
of the Fed-Ex air bill. You must call Peninsula Visa the day you
ship your documents. At that time your order will be taken. If you
do not contact us and get a work order number there will be no courier
available to process your documents. Also we will process your documents
as Rush service and additional costs will apply if you send documents without a
work order number.
