How it Works

1. Gather the documents listed below.

2. Appear in person at an Acceptance Office. (?)

3. Ship the "Sealed Package" to Peninsula Visa. (?)

4. Receive your passport.

Step 1: Gather Documents Below

 Estimated time of burden is 45 minutes.


1. Complete Online Application DS-11

  • You must complete the application online and then print it out. 
  • Handwritten applications and double sided printing are not allowed.
  • An additional copy of the application must be attached to the outside of your sealed package.

Use the slideshow below as a "how to" guide for completing the application.




2. One Passport Photo

  • You must include one passport photo 2x2in on a white background taken within the past three months.



3. Proof of US Citizenship

  • You must provide one of the following:
    • An original certified birth certificate issued by the city, county or state.
    • A naturalization certificate
    • Certificate of citizenship 
    • Consulate report of birth abroad



4. Identification

  • You must provide an original and one photocopy of your valid drivers license, naturalization certificate or military ID. If you cannot submit primary identification, please see Secondary Identification.




    5. Two Authorization Letters

    • You must print and sign two Authorization Letters. This letter gives us the ability to hand deliver the documents to the Passport Office on your behalf. 
    • The letters must have an original signature, they cannot be copies and one letter must be included in the sealed package and one letter on the outside of the sealed package. 
     







    6. Flight Itinerary
    • You must include a copy of your flight itinerary showing a departure date within 14 days. (?)






    • Our internal order form, please complete and send with your documents.
    • This order form does not go inside the sealed package.

    Step 2: Locate an Acceptance Office

    Click on the image below and then enter your zip code. 
    Select the location nearest to you and call to verify correct hours and location.




    Step 3: Appear in Person

    Estimated time of burden is 60 minutes.

    • You will need to bring a cashiers check or money order in the amount of $170 made payable to the, "US Department of State." A separate execution fee of $25 will be charged and in most cases can be paid by credit card or cash.
    • The Acceptance Officer will put all of your documents into a "Sealed Package" and hand it back to you. You will then need to send Peninsula Visa the "Sealed Package."




    Step 4: FedEx Documents

    • Simply click the FedEx logo below and a shipping label will automatically be generated to use for sending your Sealed Package. Print the label and place the documents in a FedEx envelope. An additional fee of $35 will be applied to your order.
