How to Replace a Lost/Stolen/Damaged Passport An appearance in person at a Passport Acceptance office is required and a mandatory step of your passport order. After gathering the documents listed below you must take them with you and appear in person at a Passport Acceptance office and get a sealed package.
When making your appearance you will pay a Government fee of $170 and a separate execution fee of $25. Make sure to have a cashiers check or money order made payable to the, "US Department of State" in the amount of $170 with you. The $25 execution fee can be paid by credit card.

A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrars office, which must be within 1 year of your birth. They will put all of your documents including your check and original legal status in a sealed package and hand it back to you.
