How to Apply for a Passport for a Minor



An appearance by both parents and the minor at a Passport Acceptance office is required and a mandatory step of your passport order. If only one parent can appear then a Special Circumstances form must be completed and notarized. 

After gathering the documents listed below both parents and the minor must take them and appear in person at a Passport Acceptance office and get a sealed package.

When making your appearance you will pay a Government fee of $140 and a separate execution fee of $25. Make sure to have a cashiers check or money order made payable to the, "US Department of State" in the amount of $140 with you. The $25 execution fee can be paid by credit card.  

Special notes regarding a Passport for a Minor:
  • Applicants must be 15 or younger
  • All children including newborns must have their own passport





  • Must be completed online, it cannot be handwritten
  • Must be printed on single sheets of paper - double sided printing is not allowed
  • Do not sign the application until you appear infront of an Acceptance Agent



2. One Passport Photo


  • Must be 2x2in on a white background
  • Must present the full head from the top of the hair to the bottom of the chin
  • Center the head with the frame



3. Proof of Citizenship


You must provide an original of one of the following:

  • Certified birth certificate issued by the city, county or state
  • Consulate Report of Birth Abroad or Certification of Birth
  • Naturalization Certificate or Certificate of Citizenship
A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrars office, which must be within 1 year of your birth.

Beginning April 1, 2011, all birth certificates must also include the full names of the applicants parent(s). For more information please see New Requirements for US Birth Certificates.

  • Print and sign two copies
  • One must be included inside of the sealed package
  • One must be attached outside of the sealed package



5. Flight Itinerary


  • You must include a copy of your flight itinerary or ticket holdings
  • Must show a departure date of within 14 days of when you get your sealed package




6. Identification


Both parents must provide their original and one photocopy of the following:

  • Valid Drivers License
  • Naturalization Certificate
  • Current Government ID (city, state or federal)
  • Current Military ID

7. Locate an Acceptance Office

8. Appear in Person



Once you've gathered all of the documents listed you need to take them to a Passport Acceptance Office and get a sealed package. 

You will need to bring a cashiers check or money order in the amount of $140 made payable to the, "US Department of State." A separarte execution fee of $25 will be charged and in most cases can be paid by credit card or cash.

They will put all of your documents including your check and original legal status in a sealed package and hand it back to you. 



9. Completed Order Form


You must include one completed order form with your documents.
