  • Your current second US passport is in your possession 
  • You have upcoming travel that requires the need for a second US passport
  • Your current 10 year passport is valid for more than 2 years

Required Documents


1. Complete Online Application DS-82

You must complete the online application, print and sign it. Handwritten applications and double sided printing are not allowed. Help Guide





2. One Passport Photo

You must include one passport photo 2x2in on a white background. This photo must be taken within the past three months, present the full head from top of the hair to bottom of the chin and the head centered with the frame. This photo must have been taken within the past 3 months and cannot be a photo used for any other past or present passport.




3. Current or Most Recent 2nd US Passport

You must submit your curent or most recent undamaged 2nd passport. Your old passport will be returned to you along with your new one.



4. Copy of Valid 10yr Passport

You must submit a copy of the information page of your 10yr passport. This is the page with your picture and passport number.

Your 10 year passport must be valid for at least 2 years. If expiring in less than 2 years, you must first renew your 10 year passport.





You must submit an original business letter with a wet signature from the company here in the US explaining why a second passport is needed.





You must print, complete, sign and include with your documents. 





You must print and sign two Authorization Letters. This letter gives us the ability to hand deliver the documents to the Passport Office on your behalf. We need the letters with an original signature, they cannot be copies.

 








You must include a copy of your flight itinerary.
