“SEALED” PACKAGE INSTRUCTIONS

Take this list with you to a Main Passport Office that has a passport agent. Tell them that you want all of your documents sealed and handed back to you. Make sure all items are put into the package to be sealed; Then call Peninsula Visa to have us pick it up or deliver it to our office or call for Fed - Ex instructions.

1 New Passport Application (create your DS 11 here) sign this application only in front of a passport agent.

Two Passport Photos 2” x 2” (Must have white background.) The Postal officer preparing your package must sign the back of one of the two photos.

Original or certified copy of birth certificate or original Naturalization Certificate or an expired passport over 12 years old.

Letter authorizing Peninsula Visa to pick-up your passport upon completion. You must sign and date this letter. (click here for letter)

Itinerary showing your travel date.  The travel date must be within 2 weeks of your planned departure.

Take your Identification (Drivers License, State Identification Card or other legal ID).

Two checks totaling $160.00. One made out to Passport Services for $135.00 and one made out to the U.S. Post Office for$ 25.00. You can also take cash or an ATM Card. (This fee reflects the expedited passport fee of $60.00 for processing in less than 10 weeks.)

If some Postal employees balk at preparing this package, tell them to refer to page 31 of the Passport Agents Manual “Hand Carrying of Executed Applications”.
