Forms and documents

Prepare the child's citizenship evidence, a compliant photo, Form DS-11, parent or guardian ID, and evidence of parental relationship. Both parents usually appear in person.

Timing and pricing

Child passports have their own government rules and validity period. Government fees, shipping, and any acceptance appointment timing are separate from service fees.
