Keep the damaged passport, plus citizenship evidence, photo ID, a compliant photo, and Form DS-11. The original damaged passport should be included when available.
Timing and pricing
A damaged passport is not renewed through the standard renewal path. Government fees and shipping are separate from the service tier.
